I usually play well but often knock myself out when pressured by time.

Example #1

After Black's 36. -, Nxd4 move, I over-complicated the game by playing 
37. Qf4, N8e6 38. Qg4+, Kh8 39. Bxd4, etc. and lost(!) it 20 moves later.
Simply taking the Knight with the Bishop would lead to a quick end:
37. Bxd4, Kxh6 38. e6, f6 39. g4 followed by g5

I had a good plan with 37. c4, bxc 38. bxc, dxc 39. Nxc4  
Hardly any time left on my clock, I suddenly realized that after 39. Nc4x 
Black can play 39. -, Rxc4 and if I take the Rook, 40. -, Nd6+ wins my Rook. 
I failed to see that after 39. -, Rxc4 I have 40. Rxe7+ capturing Black's Bishop. 
So, instead of 39. Nxc4 I played 39. Kc3 and then quickly lost the game.

After sacrificing my Bishop in move 20, I struggled for almost another 30 
moves trying to checkmate White's King. By move 20, both of us had used 
up most of our time, and the game got somewhat wild. 
After 37. -, Qh3+ 38. Kf2, Bh4+ 39. Kg1 I played 39. -, Bd8. White had only 
seconds left, playing with the 30 seconds added after each move, so I thought 
this move would surprise my opponent and would cause her to lose on time. 
She was probably expecting 39. -, Qg3+ ... and that is exactly what I should have played. 

40. Kh1 or g1 gives up the Rook on d2. 40. Bg2 is at least a draw. 
After 40. Rg2, Qe1+ 41. Kh2, Bd8 is draw by the Qe1-Qh4 checks. (41. -, Bf6  
is, of course, good for White: 42. Rh7+, Kxh7 43. Nxf6+ and then mate on g8.)

This was a rather dull game until White's King advanced to f6 in move 73, 
allowing me to push my c pawn. White had no defense because I was lucky
to have all the pieces in the right places.
